I certify that I completed this application and all information I have provided to apply for and secure work with King Systems is true, complete and correct, to the best of my knowledge. I understand that any information provided by me that is found to be false, incomplete or misrepresented in any respect will be sufficient cause to (i) cancel further consideration of this application, (ii) revoke an offer of employment, or (iii) discharge me from the Company’s service.

I authorize, without reservation, the company, its representatives, employees or agents to contact and obtain information from all references (personal and professional), employers, public agencies, licensing authorities and educational institutions and to otherwise verify the accuracy of all information provided by me in this application, resume or job interview. I understand that an investigation may be made and information may be obtained through interviews with personal references and past employers, through a credit check, a criminal history check and/or a driver’s record check. If King Systems decides to obtain a consumer credit report, I understand that King Systems will provide, at my request, the name and address of the reporting agency so I may obtain from such reporting agency the nature and substance of information contained in such report.

I understand that, if I am offered a job, as a condition of beginning my employment, I may be required to undergo a physical examination and drug screen, and I hereby authorize any doctor, hospital, clinic, laboratory and/or other medical facility to furnish any medical information with reference to me as may be necessary in conjunction with that examination and related considerations. I further understand that my failure to promptly consent to and cooperate with any such examination or screen, any attempt to tamper with, or actual tampering with, any specimen, or providing false information or omitting information requested, will be communicated to the company and will result in the termination of my employment or the rescission of any offer for employment if I am not employed.

If hired, I understand that I am free to resign at any time, with or without cause and without prior notice, and the company reserves the same right to terminate my employment at any time, with or without cause and without prior notice, except as may be required by law. Some of the manufacturing processes at King Systems employ the use of Methylene Chloride (MC) as a bonding agent. If you work in a position that utilizes MC, you will be required to participate in the company’s training and Medical Surveillance Program which mandates physicals and blood tests (paid for by the company) at intervals, including those intervals designated by OSHA standards for MC. In addition, please be aware that MC has been shown to cause carcinoma (cancer) and have other adverse effects in laboratory animals. The Occupational Safety and Health Administration (OSHA) has determined that persons exposed to certain levels of MC are at increased risk of developing cancer, adverse effects on the heart, central nervous system and liver, and skin or eye irritation. Exposure may occur through inhalation, by absorption through the skin, or through contact with the skin. Accordingly, from an employee health and safety perspective, the company requires that employees utilize certain procedures and personal protective equipment supplied by the company when utilizing MC. Also, certain employees with sensitivity to MC such as those with pre-existing lung ailments may experience discomfort or other health issues when working with MC. The company is willing to make reasonable accommodations as required under by law.
